Some Baldur’s Gate 3 players are reporting new issues involving the game not realizing they have important quest items in their inventory after the release of Patch 16. Thankfully, the solution looks to be as simple as disabling certain mods.
Ironically, one of the major issues corrected by BG3’s latest update was that some people managed to completely soft-lock their Honor Mode runs by forgetting to bring mission-critical items when they needed them most. After the fix, you can now access certain items at any time, no matter where you put them.

Sadly, if the game randomly refused to realize that you had things like Orphek’s Hammer, Art Kara’s Broken Lute, or even some magic items that you needed to feed Gale, then this would become quite a Meaningless question. As stated in a recent post on the gaming subreddit, this is exactly what happened to many players after updating the game to install the new patch.
Considering how serious this issue actually is in Baldur’s Gate 3, it’s no surprise that some of those affected were quick to try and figure out whether the patch was actually the cause of the issue.They’ve even put together a very helpful l ist of workarounds for some important projects affected
These strategies include climbing up to the balcony (how romantic) and doing some parkour in order to avoid your invitation to Raphael’s boudoir, but not being picked up, and having to manually put items into Gail before switching to Gail The wizard can be obtained from the inventory in order to satisfy his thirst for magic.
The good news is that after some troubleshooting and one player reaching out to Larian support for guidance, it looks like the possible root cause of many of these cases has been found. The post’s author wrote in an update: “The problem seems clearly to be mods, with at least 12 replies below stating that removing some or all mods fixed this quest item bug.”
“A common culprit seems to be SildurFx’s ‘Party Limit Gone’, but some people have reported that other mods are causing the issue as well,” they added. So if you want to make sure you avoid running into any issues, and also want to see if Larian and/or some of the modders behind the affected titles can correct the issues, you may have to go back to vanilla BG3 for the time being: Fix One two.
The good news is that if you’ve been waiting for a permanent solution to the save issues that have been affecting your Xbox BG3 for the past few weeks, a firmware update due out next week should resolve your issues.
