This might surprise you, but Fallout: London – the massive mod for Fallout 4 – has been extremely popular among Fallout modders outside of the FOLON team. Some of them are big fans of New Vegas and have even started bringing that game’s faction reputation and disguise systems to the UK.
To be fair, some of them have done some obvious things in terms of modes, like giving your character a cure for a severe dog food addiction, and it looks like they’ll have to wait a while before they can enjoy Fallout: London’s Wild Card main quest arc, so let them have some fun in the meantime.

Invatorzen’s “faction reputation and disguise system” looks to be the most comprehensive attempt yet at letting you officially confirm that you’re the most maligned individual in London, rather than just knowing it because every NPC won’t stop shooting at you.
As you’d expect, it adds a reputation component to your Atta-boy, letting you see whether your actions for or against a particular faction have earned them fame or infamy, with tiers of love and hate that are identical to the language of New Vegas – Adore, Neutral, Hate, and so on – as shown below. It also includes a New Vegas-style faction disguise system, allowing you to don some of the threads you find and play the role of a clever infiltrator.
“A few years ago I developed the faction reputation and disguise system for Fallout 4 – unfortunately I’ve played the game on PS4, PS5, and PC so many times that I lost motivation to develop the mod,” he explained. “However, after the release of Fallout: London, which is smaller in scale than Fallout 4, I thought I’d revive the mod so I can bring the system to London.”
At the moment, the mod appears to still be a work in progress, with individual towns/settlements yet to be added to the reputation system, and Invatorzen has it set up so that they can take suggestions on which disguises and quest reputation rewards should be added as they continue development.
The modder also adds that their creation “will not grant you faction reputation or fame earned from previously completed quests or NPCs killed,” so you may need to resave to use it properly.
If you’re just interested in being able to wear faction disguises yourself, then another mod, DanElwood’s “Faction Disguise System”, might be worth a look.
