The battle between Itachi and Pain would undoubtedly be anything but predictable. They were both two of the highest level ninjas to ever exist in the Naruto universe.
But although there is no absolute answer on this subject, there are very important points from which we could draw our own conclusions.

Pain had more weaknesses than Naruto took advantage of
During the invasion of Konoha many ninjas sacrificed their lives to collect important information regarding Pain. This data would later come to Naruto to help him combat him, especially regarding his powers and weaknesses.

A weakness that all the bodies shared, and perhaps the most important for this issue, is that of a shared vision. This could be interrupted by smoke or coverage. And while they gave Pain potentially infinite vision, they also left the Akatsuki open to genjutsu.

Itachi, the slowly fading light
we all know Itachi Uchiha like a genius of those who only seen every few generations. However, despite what many think, he was not the god of Naruto, and for many reasons.
Let’s start because the prodigy was terminally ill and only had a few years to live. To this we must add that he was going blind, so that each fight subtracted a percentage of use from his abilities. In his match against Sasuke he showed us that he didn’t have much time left anyway.
Itachi easily ran out of resistance. He was exceptional at taijutsu, but his body was weak. And his strongest attacks involved the Mangekyo Sharingan, which drained a significant part of his power. Unlike Naruto, he couldn’t use his Tsukuyomi, Amaterasu, and Susano’s aces multiple times in the same match.
It must be added that as a member of the ANBU from a young age, he learned to depend (perhaps too much) on information. Itachi was not used to fighting blindly, and he actually knew very little about Pain, Nagato, and even Madara Uchiha.
The renegade Uchiha believed in violence to resolve conflict, as he demonstrated by murdering his entire clan. But despite that, he was a bit soft-hearted and, unlike narutocould have been convinced by Pain’s vision of peace as long as he didn’t mess with Konoha.
Itachi vs Pain: Naruto’s Strongest Akatsuki

Itachi would have to put Pain’s Jutsu-absorbing body into genjutsu in order to cast an Amaterasu to the rest, who will still be able to cover themselves with summons or quickly escape. All this while somehow surviving the missiles, monster, and massive Shinra Tensei that held back even Kurama with Nine Tails activated.
Even if he could do it by taking cover with his Susano, he would most likely Itachi will end up dead. The Naruto series has established well that Itachi suffers a lot after using his best attacks. He would hardly have time to find and defeat Nagato before he fell. Pain, despite having less talent, intelligence, and being worn down; had much more power and stamina.
The greatest proof of this is that if he had been able to defeat Pain, he himself would have defeated Akatsuki. Itachi dedicated her life to protecting Konoha, and Pain utterly tore her apart. The Uchiha wouldn’t have let this happen if he could have prevented it, even with an Izanagi. And that, in my opinion, is what Kishimoto tried to convey to us in Naturo.
